Have you tried filling in this survey, a link to which was sent by email?

My answer for the very first question is "Other (please specify)" and I specify in the text box provided; fill in the rest of the first page and hit the Next button, only to be returned the error for the first question, "This question requires an answer." Presumably, the "Other (please specify)" should be accompanied by a radio button…
