Icons aren't accepted in stand alone PC application

When I try to choose an .ico file, i get a message saying stuff like you need 16x16 256 color etc etc... I tried creating an icon with all those dimensions but it ended up being really blurry for some reason...
any ideas? or any quick fixes

I'm using Enterprise edition :)
