Apple approval for enterprise apps

Hi, what is the procedure to get Apple's approval of an app that is to be distributed with an Enterprise license?

Does this mean that there is no difference in approval for apps distributed with an Enterprise License and apps distributed in the app store?
Are all apps distributed through the app store?
I was under the impression that under the Apple Enterprise Development program apps can be distributed from a web site.

But understand you are only permitted 500 installs by Apple with the Enterprise license (I think, these things change so fast). So it's not really anything like distributing through the app store.
